Section A: Information About This Application Service You may use this application form to apply for assessment of eligibility for Certificate of Proficiency as Able Seafarer Deck Section B: Relevant Documents Needed You will need to scan the following documents/information in soft copy before filling up the online application: General Documents Passport Passport size photo Supporting Documents Required (Ancillary courses/certificates) Certificate Title ABD TRAINING COURSE ANCILLARY COURSE COMBINED REVALIDATION VI/1, VI/2 & VI/3 (If Certificate of proficiency in survival craft & Advanced fire fighting, not revalidated every 5 years) CERTIFICATE OF MEDICAL FITNESS (including colour vision) CERTIFICATE OF PROFICIENCY IN SURVIVAL CRAFT & RESCUE BOATS (5 yearly revalidation) CERTIFICATE OF PROFICIENCY SEAFARERS WITH DESIGNATED SECURITY DUTIES (Table A-VI/6-2) ELEMENTARY FIRST AID CERTIFICATE (OR) PROFICIENCY IN MEDICAL FIRST AID FIRE FIGHTING CERTIFICATE (Basic or Advanced certificate) PERSONAL SAFETY AND SOCIAL RESPONSIBLITY Section C: Scanning Document Instruction You will need to prepare all the relevant documents in the following format before uploading to the application form Passport Size Photo Your photograph image must be JPEG or PNG format("jpg" or "png" extension) size should not be more than 150 Kilobytes dimension of the photo image has to be 400(width) by 514 pixels(height) in color and against plain white background without shadows taken within the last 3 months show the full face and without headgear (Headgear worn in accordance with religious or racial customs is acceptable but must not hide the facial features) Other relevant documents Your other documents must be JPEG, PNG, PDF or Microsoft Word format Size should not be more than 1 Mbytes Document file naming Your uploaded file name must be not more than 20 characters should only be alphanumeric characters(A-Z, and numbers) should not have any special characters such as @, ~, *, <, >, &, ', %, :, .., \ Section D: Things To Do After Submission At the end of the online submission, please follow the instructions on the acknowledgement page Print application acknowledgement page Print a copy of the application acknowledgement page and take note of the application reference number. You will need to provide the application reference number and your passport number when contacting Maritime and Port Authority of Singapore regarding the application. Application fee For Marinet users those who subscribed to AAF including Tanker Endorsement, the charges are included in the AAF scheme. Separate bills will not be issued.
