Application for Tanker Endorsement Immediate Responsibilities (TEIR-Higher Level) Important Notes: (a) This service will take about 20 minutes to complete. (b) Please read the below sections before proceeding to submit your application. Section A: Information About This Application Service You may use this application form to apply for endorsement to Certificate of Competency to serve on Tankers as Level 2- Immediate responsibility for loading, discharging, care in transit, handling of cargo tank cleaning or other cargo related operations (Management level) Use this application if you are holding a Certificate of Competency(COC) or pending COC application approval from MPA(COC) or a holder of Certificate of Proficiency (COP)/Certificate of Service (COS) Section B: Relevant Documents Needed You will need to scan the following documents/information in soft copy before filling up the online application: General Documents Passport Passport size photo Certificates Required Certificate of Competency (COC) Authentication of COC (if you are holding COC that is not issued by Singapore) Certificate of Proficiency or Certificate of Service Courses/Training Required Certificate Title Type of Tanker you are applying Oil Chemical Liquefied Gas (LG) ADVANCED FIRE FIGHTING CERTIFICATE (5 yearly revalidation) ✓ ✓ ✓ ADVANCED TRAINING FOR OIL TANKER CARGO OPERATIONS CERTIFICATE ✓ ADVANCED TRAINING FOR CHEMICAL TANKER CARGO OPERATIONS CERTIFICATE ✓ ADVANCED TRAINING FOR LIQUEFIED GAS TANKER OPERATIONS CERTIFICATE ✓ Sea Service Records/Onboard Training Applicant can either have 3 months of sea service or 1 month of approved onboard training for each type of tanker endorsement(Oil, Chemical, LG). Sea service must be within the last 5 years. Sea service/onboard training must match with the type of tanker that you are applying. For example, if you are applying for oil tanker, you must have at least 3 months of sea service or 1 month of approved onboard training on oil tanker. Section C: Scanning Document Instruction You will need to prepare all the relevant documents in the following format before uploading to the application form Passport Size Photo Your photograph image must be JPEG or PNG format("jpg" or "png" extension) size should not be more than 150 Kilobytes dimension of the photo image has to be 400(width) by 514 pixels(height) in color and against plain white background without shadows taken within the last 3 months show the full face and without headgear (Headgear worn in accordance with religious or racial customs is acceptable but must not hide the facial features) Other relevant documents Your other documents must be JPEG, PNG, PDF or Microsoft Word format Size should not be more than 1 Mbytes Document file naming Your uploaded file name must be not more than 20 characters should only be alphanumeric characters(A-Z, and numbers) should not have any special characters such as @, ~, *, <, >, &, ', %, :, .., \ Section D: Things To Do After Submission At the end of the online submission, please follow the instructions on the acknowledgement page Print application acknowledgement page Print a copy of the application acknowledgement page and take note of the application reference number. You will need to provide the application reference number and your passport number when contacting Maritime and Port Authority of Singapore regarding the application. Pay application fee For the application to be processed, please pay SGD$25 non-refundable application fee to 'Maritime and Port Authority of Singapore'.
